Access to Care Video

Provides ongoing primary care for complex patients. The clinic provides consistent longitudinal care to unattached patients as well as transferring them to community practices.

Working with local family doctors to take the Live 5-2-1-0 message​ into the community through initiatives such as Walk With Your Doc, Be Active Every Day, and the local play boxes.

Community Engagement

Free, confidential, drop in medical and mental health services for youth and young adults ages 12-26.

Chilliwack Youth Health Centre

The goal of the Child and Youth Mental Health and Substance Use Collaborative is to increase the number of children, youth and their families receiving access to INTEGRATED mental health and substance use services/supports throughout the province.

CYMHSU Collaborative

The Chilliwack Division of Family Practice launched its Hospital Care Program in July 2010.

Hospital Care Program

The GPSC is introducing the ​​RCI to improve care for ​seniors. ​Fewer physicians are providing services to seniors in care so the GPSC is supporting local groups of physicians through the ​RCI to build sustainable solutions.
