What is the Patient's Medical Home (PMH)? The PMH is an evidence-based practice model designed to deliver essential elements of full-service primary care. Family physicians and nurse practitioners, supported by a team of nurses and other healthcare professionals, work collaboratively to provide a range of services that meet the diverse needs of patients.

Introducing Primary Care Networks (PCN)

Introducing Primary Care Networks (PCN) To realize our vision, we are implementing Primary Care Networks (PCN) across British Columbia. These networks simplify the healthcare system, focusing on team-based primary care services and enhancing capacity and patient access. PCNs bring together health authorities, physicians, nurse practitioners, nurses, allied health professionals, and community providers to ensure that all members of the community receive the primary care they require.

Our Commitment to Barbara Starfield's 4Cs of Primary Care Through the establishment of PCNs with PMHs as their foundation, the Fraser Northwest Division of Family Practice and primary care providers in New Westminster and the Tri-Cities are dedicated to delivering primary care that embraces Barbara Starfield's 4Cs:

  1. First Contact: Our primary care providers serve as the initial point of contact for patients, ensuring they have access to timely and comprehensive care.
  2. Longitudinal Relationship: We are committed to building long-term relationships with our patients, fostering trust, continuity, and personalized care.
