Primary Objectives

• to educate and motivate Kootenay Boundary GPs to reduce polypharmacy within residential care settings, as well as with patients living independently
• to support staff in residential care facilities in understanding the risks of polypharmacy and to promote medication reduction

Secondary Objectives

• to track medication counts and other data related (ie falls, use of restraints)
• to improve the med review process
