Jacob Hart leads others to see possibilities and take practical steps forward in times of adversity and change. For the last 6 years, Jacob has worked with family physicians and their staff across the Lower Mainland through Fraser Health’s Practice Support Program. Jacob brings extensive facilitation, coaching, and quality improvement experience and training, and has supported a significant number of QI projects in primary care. He believes that any individual or team can improve once motivated and properly supported, and that leadership is not an innate quality, but rather a skill that can be developed and grown over time. Jacob brings an abundance of creative energy and enthusiasm to each project, and is very much looking forward to carrying his experiences from the Lower Mainland back to his hometown of Victoria, BC to help support the development of Primary Health Care Networks. He previously taught English and writing and completed a Master’s of Arts degree at the University of Victoria in 2012.
