Physicians in Surrey and North Delta have come together to provide a virtual clinic where patients who do not have a regular family doctor can receive care, including regular medical care and COVID-19 assessments. 

**Please DO NOT visit a clinic in person.**

What to Do If You are Concerned You May Have COVID-19

 

Complete the COVID-19 Self-Assessment Questionnaire

This province-wide survey will help you determine what steps you need to take. Simply answer the questions as accurately as possible. At the end, you will be directed towards the appropriate healthcare service.

If the questionnaire prompts you to call 811, you have the option to book a virtual appointment with a family physician.

  • Contact your family physician to see if they are offering virtual assessments. 

 

⇒ Schedule an Appointment With Our Virtual Clinic **If You Do Not Have a Family Physician**

If your family physician is not offering virtual or phone assessments OR you do not have a family physician, complete the steps below:

REMINDER: A virtual assessment is NOT a COVID-19 test.

Step 2: Schedule Your Appointment

Once you have completed the form, we will contact you to schedule an appointment.

We will email you a link -- this is how you will access your appointment. 

Step 3: Start Your Appointment

When it is time for your appointment, click the link in the email from our clinic.

We recommend joining the virtual waiting room a few minutes before your scheduled appointment.
