SHARED CARE

Shared Care is a province-wide committee that "supports family and specialist physicians to improve the coordination of care from primary to specialist services".

Through Shared Care, Family Physicians, specialists and other partners have come together to lead over 450 projects aimed at building team-based approaches to care and improving communication between patients and their families, primary care physicians, specialists and other health professionals.
