Care of the Elderly

Improving care for elderly patients has been a strategic priority for the Victoria Division since its inception.

The Care of the Elderly working group has been key to improving attachment and access to quality, primary and community care for this population. The success of the TORCH (Toward Optimal Residential Care Health) model and implementation of the Long-Term Care Initiative (formerly named Residential Care Initiative) has created a stable, high functioning model of team-based care in Victoria with a high level of both patient and provider satisfaction.

With a focus on how to best support physicians caring for frail seniors in the community, the working group evolved into a joint Victoria CSC (Collaborative Services Committee) working group to collaborate with Island Health Community Health Services (CHS) in Victoria. One of the first outcomes was a new Physician Connector Service to bridge communication gaps between family practice and CHS. The new service improved access to CHS and created a more efficient family practice environment.

Focus on the Victoria Primary Care Network

The work then focused on supporting the Victoria Primary Care Network. The group organized a series of neighbourhood development sessions with family physicians and CHS teams within designated geographic areas, creating a foundation for collaborative care.
