To help the frail elderly and patients with complex needs better understand their medication and ensure an optimal prescription mix, our Division completed a prototype program that is now funded by Fraser Health to hire a clinical pharmacist to conduct medication reviews at a family doctor’s request.

Physicians can request assistance with MMR from a pharmacist at PAH by either contacting the pharmacist directly or writing a request in the Physician's Orders (i.e. "pharmacist to please review medications") and indicating specific medication concerns.
