UpToDate is an online clinical decision support resource, available to Divisions of Family Practice members for use in individual practices at no cost to the individual physician. Visit your local Division website to become a Division member and access more than 10,000 peer-reviewed topics in 21 specialties from international and Canadian authors, as well as drug information, medical calculators, and patient information sheets. All UpToDate topic searches are CME eligible for registered users.

To access UpToDate log in at the WRSS Division site and click on the UpToDate menu item on the top navigation bar. For more detailed instructions, see how to access UpToDate or contact the UpToDate support team for assistance.

Pathways is a web-based resource designed to streamline and optimize patient referrals. Pathways was developed by the Fraser-Northwest Division of Family Practice, where it was piloted successfully and found to offer genuine solutions to GP-specialist referral issues.

Homebound Referral Form

Dr. Arja Moreau is available to assume MRP for any homebound frail seniors that live in the White Rock-South Surrey area that you are not able to continue to provide care to.

If you wish to refer a patient, please complete the following referral form and fax to the Primary Care Access Clinic at 604 531 3351. Dr. Moreau will contact you to discuss handover.

Rapid Access Clinic Referral to the Vine Youth Clinic

Rapid Access Clinic for Child Psychiatry (15455 Vine Ave, White Rock) is a service for children/youth who are 18 or younger and who live in the White Rock/South Surrey Division of Practice area. Dr. Carol Ann Saari, Child Psychiatrist, is available to provide consultations and limited follow-up for all referring GPs in the area. The clinic days are every second Thursday for a full day. 

Referrals may be faxed to 604 531 8308, and should clearly indicate “RAC referral”. You can also access the referral form on Pathways.

Alzheimer Society of BC First Link

First Link is an early intervention service designed to connect individuals and families affected by Alzheimer's disease or another dementia with services and support as soon as possible after diagnosis. Formal referral from physicians and health professionals allows for proactive contact with individuals and families. Individuals and families may also self-refer.
