Primary Care Access Clinic
Primary Care Access Clinic at the Centre for Active LivingIn
partnership with the Fraser Health Authority and Peace Arch Hospital and
Community Health Foundation, our Division opened the Primary Care
Access Clinic to support patients without a regular family doctor who
need care following a hospital stay.
As part of this
post-hospitalization care, our clinic team connects unattached patients
with appropriate community services, such as home health, mental health
and addictions, and complex case management. We also help match patients
to new doctors in our community or those accepting new patients.
As well, the clinic provides long-term care for some patients.
The Clinic TeamNurse practitioners provide the majority of care at the clinic, with input
from a family doctor when needed. Specialists are also available to
assist patients with mental health and addictions concerns. The clinic
has a dedicated medical office assistant who schedules appointments and
provides clerical support.
