Long-term Care Initiative

 

The provincial Long-term Care Initiative (LTCI) is a physician initiative designed to improve care for older adults in long-term care (LTC) homes.

The North Shore LTCI was launched in June 2016 and 11 LTC homes are now involved. 25 local family physicians are participating in the program. The Division works with Vancouver Coastal Health, LTCI family physicians, and LTC home management to improve care for patients living in LTC homes.

LTCI Physicians can be compensated through the Family Practice Services Committee (FPSC) based on meeting the 5 best practice expectations listed below.
