Physiotherapist

The PCN Physiotherapist offers short-term services that assist individuals affected by injury, illness or disability to restore movement and function. Treatment may include one-on-one support, group education and connections to local community resources.

The PCN Physiotherapist is a part of the PCN Chronic Disease team that work together to support the individual's overall health.

Examples of areas that the PCN Physiotherapist can support include:

  • Support and promotion of healthy lifestyle behaviours through physical activity
  • Chronic disease management (e.g., diabetes, pain, COPD, heart disease, etc.)
  • Self-management education for recovery, pain relief, injury prevention, improved physical movement and mobility (e.g., falls prevention, etc.)
  • Graduated home exercise programs with a focus on strength, balance and physical abilities
  • Addressing the impact of injury, pain, disease and disorders on functional movements, activities of daily living, participation and quality of life

Eligibility

  • Individuals must be referred by their family physician or nurse practitioner
  • 18 years of age or older
