Social Work

PCN Social Workers offer short-term, one-on-one counselling, navigation and problem-solving interventions. This can include helping individuals gain access to appropriate information and community resources. Services can be provided in-person or virtually depending on individual needs.

Examples of areas that a PCN Social Worker can support include:

  • Housing
  • Coping strategies
  • Isolation/support networks
  • Advanced care planning
  • Caregiver responsibility 
  • Financial hardship
  • Safety/risk
  • Service/system navigation
  • Life changes
  • Change in health needs

Eligibility

  • Individuals must be referred by their family physician or nurse practitioner
  • 18 years of age or older
